applicants should complete the following instructions
once you have received your application by mail
or fax.

Complete the application form.
2. Submit (3) photos. (2 of the inside of the
store and 1 of the outside.)
3. Submit a listing of current vendors.
4. Include a current financial statement.
Once
your application has been reviewed, all applicants
will receive a decision by mail.
Please
note: Completed applications usually take 3-4
weeks for review from the date of receipt.
